PRIVACY NOTICE

This notice reaffirms Community Contacts, Inc.’s realization of and respect for the privacy expectations and rights of our clients regarding financial information and other related information, which the company has or gathers in the normal course of business. It is intended to provide guidance to Community Contacts, Inc. (CCI) personnel as well as reassurance to our clients. Employees of CCI are required by law to maintain confidentiality and to safeguard client records and information. Staff meetings and client information are private and will be treated as such. Client files and records are handled discreetly and filed appropriately to retain confidentiality. Additionally, client files are shredded according to pertinent programmatic requirements and agency document retention schedules.

Only authorized persons will have access to case/client information. Confidential information includes that which is obtained either through written record or through daily interaction with the person, including the fact that an individual is served by CCI. Client information may only be released as necessary to effect and administer your assistance through CCI. In addition, information may only be provided with your consent, or as permitted or provided by applicable laws, including the Illinois Freedom of Information Act (FOIA) and the Privacy Act of 1974. The obligation of the employee to maintain the confidentiality of client information shall survive the termination of employment of the employee, regardless of the reason or reasons for termination of employment with CCI.

We do not sell or otherwise make available any information about you to any third parties for marketing purposes. Any breach of confidentiality is a serious matter and may result in disciplinary action up to and including termination.
